To schedule a biometric screening with Quest Diagnostics, please follow these steps:
- When viewing the home dashboard of your Sonic Boom account, find & click on "My Health"
- Scroll down and select the "Enroll in biometrics" button
- You'll be redirected to your Quest Diagnostics account
- Accept their terms & conditions
- Confirm the information they have on file for you - ensure there's a valid email, phone number and address
- Choose your screening method:
- Quest Physician Results Form - have a screening with you primary care doctor.
- Quest Patient Service Center - visit a service center near your zip code to complete a screening.
- Home Test Kit - complete a screening in the comfort of your home.
Note -- for Sonic Boom Wellness to receive your biometric screening record, the appointment MUST be made within your personal Quest account (via the steps above). Should you call Quest directly or create an account through their website directly, or walk into a Quest Patient Service Center near you, we will not be able to receive your screening record.
