How do I complete a Health Quality Assessment?

When this feature is enabled, the Health Assessment widget, will appear within the My Health section of your Sonic Boom Wellness account.  The assessment can be accessed either using a computer or the Sonic Boom Wellness app. 

  • When viewing the home screen of your Sonic Boom account, select the My Health section (heart icon with a pulse symbol)
  • Under Health Assessment, select the "start now" link
  • You'll be re-directed from your Sonic Boom account, to Health Aware; the vendor we've partnered with to offer the assessment. 
  • Answer the questions to get your personal wellness score – it should only take you about 10-minutes!

Tip - use the results from your latest biometric screening to help answer the questions in the health assessment.

Not seeing the "start now" link?

It's possible the time-frame to complete the Heath Quality Assessment has ended.  For more information about this deadline and your next steps, please contact the Sonic Boom Support Team.
